	Used Vehicle Inspection Checklist Perform the following inspections within 30 days prior to the registration of a Used Vehicle Contract. Initial Inspection, Under Hood, Under Vehicle and Road Test Checklists will be your responsibility to find a Ford dealer and pay for the Inspection. Request that the inspecting Dealer log the inspection with Ford. Then email or fax the completed passed Inspection Form and a copy of the paid Repair Order to info@factoryplans.com or to 269-585-6004 fax. Also, a vehicle that has been declared a total loss by an insurance company doesn't qualify for any plans. Thank you.
	Customer Transfer Waiver This form should be used to transfer Ford Protect Extended Service Plans to new owners.
